Injection Prostaglandin. Prostaglandins are medicines used to stimulate uterine contractions and ripen the cervix during labor or to control bleeding after delivery.
Common types: Dinoprostone PGE, Carboprost PGF alpha, Misoprostol PGE analog often tablet form but used in obstetrics.
Strength: 1. Dinoprostone injection, usually 5 mg per ml. 2. Carboprost injection, usually 250 mcg per ml.
Action: Stimulates uterine contractions, helps cervical ripening, controls postpartum hemorrhage PPH by contracting the uterus.
Mechanism of action: Prostaglandins act on smooth muscles of the uterus and increase intracellular calcium, cause strong uterine muscle contractions, soften and dilate the cervix.
Indications: Induction of labor, cervical ripening before labor, medical termination of pregnancy, postpartum hemorrhage due to uterine atony. Nurses' responsibilities: Before administration, check doctor's order, assess fetal heart rate and uterine contractions, check maternal vital signs, explain procedure to patient. During administration, monitor uterine contractions, fetal heart rate, maternal BP, pulse, temperature. Watch for uterine hyperstimulation. After administration, observe for postpartum bleeding, monitor side effects, document drug dose, route, and patient response. Important points: Excess dose may cause uterine rupture.
Carboprost should be used carefully in asthmatic patients. Continuous fetal monitoring is often required during labor induction.
